Why Many Couples Get Stuck Before They Get Pregnant

For many couples, months turn into years of irregular cycles, scans, supplements and waiting, without anyone sitting down to address the root causes. Hormonal imbalances, thyroid, insulin resistance, stress and nutrition usually don't show up in a 15-minute appointment, so they quietly stay in the way.

At Fertilia, we work alongside your gynaecologist by addressing the whole picture, not just prescribing medication. Many women have conceived naturally through our programs, and many more have used the program to get their cycles, hormones and metabolic health into shape so the next step (natural conception, IUI or IVF) actually has the best chance of working. (Already in an IVF cycle? See our dedicated IVF Support Program.) Dr. Suganya Venkat's approach combines evidence-based medicine with lifestyle and nutritional guidance.

Common Questions

About Conceiving, Honestly Answered

By Dr. Suganya Venkat, OB-GYN with 15+ years of clinical experience.

Can I get pregnant naturally with low AMH?

Yes. A low AMH means fewer eggs in reserve (not poor-quality eggs), and on its own it does not mean you cannot conceive. AMH mainly predicts how the ovaries might respond to IVF stimulation; it is a weak predictor of natural conception. Many women with low AMH conceive naturally, especially when ovulation is happening and the other pieces (thyroid, prolactin, the tubes, and a partner's semen analysis) are in order. What low AMH does tell you is that it is worth getting a clear picture sooner rather than later, so your decisions are based on your own body, not a single number.

How long should we try before seeing a doctor about fertility?

The usual guidance is 12 months of regular, unprotected intercourse if you are under 35, and 6 months if you are 35 or older, at which point an evaluation is more useful than more waiting. But you do not have to reach that mark to ask. If your cycles are irregular, absent or very painful, if there is known PCOS, endometriosis, thyroid disease or previous pelvic surgery, or if you simply want to understand your baseline, an assessment is reasonable at any point. Earlier information turns out to be reassuring far more often than it is alarming.

Can I conceive naturally with PCOS?

Yes. PCOS is one of the most common and most treatable reasons conception takes longer. The core issue is usually irregular or absent ovulation, and ovulation can very often be restored: for many women through the insulin-resistance and lifestyle pathway, and with ovulation-induction medication guided by your gynaecologist where it is needed. A large share of women with PCOS conceive naturally once their cycles become regular. Our step-by-step PCOS conception guide walks through exactly how.

Does fertility really drop after 35, and what are my real chances?

Fertility does decline gradually with age, but the popular numbers are far scarier than the real ones. A healthy 35-year-old still has a meaningful monthly chance of conceiving, and most couples in their late 30s who try for a year do go on to conceive. Age affects egg quantity and quality; it does not slam a door shut. The honest message is not 'hurry'; it is 'get a clear picture of your own body so your decisions are based on your numbers, not a headline.'

Do I actually need IVF, or can I conceive without it?

Most couples who arrive worried about IVF do not end up needing it. IVF is the right tool for specific situations: blocked tubes, significant male-factor, advanced age with very low reserve, or after simpler approaches have had a fair trial. For everyone else the path often runs through treating the underlying issue (PCOS, thyroid, prolactin), timed intercourse, or IUI first. A proper evaluation tells you which group you are in, so you neither delay IVF when it is genuinely the right call nor undergo it before you need to. Our OB-GYN decision framework lays this out plainly.

What does a fertility workup actually involve?

A sensible first-line workup is simple and largely non-invasive: blood tests around your hormones and ovulation (including thyroid, prolactin and AMH), a pelvic ultrasound to look at the ovaries and uterus, a tubal-patency test (HSG) when indicated, and, importantly, a semen analysis for your partner, since male factor accounts for roughly a third to half of all cases. It is usually done over a cycle or two and gives you both a clear, shared starting point. Our honest fertility-workup guide explains each test and what the results mean.
